Pancoast tumor. A 53-year-old man with a 50 pack-year history of smoking began experiencing upper back pain for several weeks. PA chest radiograph shows asymmetry of the apices (superior sulcus). In 1924, Pancoast (4) described a syndrome the outstanding features of which were an apical lung tumor, demonstrable roentgenographically, referred nerve phenomena in the upper extremity on the involved side, and cervical sympathetic paralysis suggesting a spinal cord tumor, cervical rib, or vertebral neoplasm. This patient presented for a cervical MRI for assessment of arm pain. The above coronal T1-weighted image shows an intermediate intensity mass at the right lung apex, with evidence of invasion of surrounding soft tissues.

Since Pancoast original reports, the terms "Pancoast tumors," "Pancoast-Tobias tumors," "superior sulcus tumors," or "superior pulmonary sulcus tumors" have been applied to neoplasms located at the apical pleuropulmonary groove, adjacent to the subclavian vessels (figure 1) [ 2-4 ]. Strictly defined, Pancoast tumors are non–small cell carcinomas that originate in the lung apex and that often cause signs and symptoms known collectively as Pancoast syndrome. Pancoast tumors form at the very top of either lung.
